Improvement Advisor

This is a fully remote position, open to applicants in Utah.

📋 Description

• Assist organizations in navigating quality enhancement, population health, and the planning and execution of Community Care Hub initiatives.

• Mentor partners on data gathering, analysis, and strategies for performance enhancement.

• Evaluate clinic readiness and create, as well as implement, workflows for clinical-community referrals, enrollment, and closed-loop referrals.

• Develop and apply evidence-based interventions for quality improvement.

• Organize and facilitate learning collaboratives, training sessions, and partner engagement meetings.

• Collaborate with interdisciplinary teams to enhance care coordination, establish clinical-community connections, promote community involvement, and drive population health efforts.

• Aid in the onboarding of clinics, testing workflows, executing processes, and continuously enhancing Community Care Hub referral systems.

• Monitor and record activities utilizing CRM and project management tools.

• Offer consulting and technical support to hospitals, practices, and community-based organizations in rural Utah.

⛳️ Requirements

• Bachelor’s degree in public health, healthcare, or a similar field.

• Over 4 years of experience in healthcare, particularly in quality improvement.

• More than 2 years of project management experience.

• Comprehensive understanding of quality improvement techniques, healthcare terminology, and clinical workflows.

• Exceptional communication and partner engagement abilities.

• Preferably, prior experience in outpatient clinic or hospital operations.

• A Master’s degree (MA/MS) in Public Health, Nursing, Healthcare Administration, Public Policy, or a related discipline is preferred.

• Experience in healthcare operations, clinical workflows, care coordination, population health, or quality improvement is advantageous.
