This is a fully remote position, open to applicants in North Carolina.
• Implement person-centered planning techniques and strategies to collect information, allowing for a deeper understanding of the members being supported and effectively addressing their individual needs.
• Educate and assist members in understanding and exercising their rights and responsibilities.
• Conduct discovery activities (information gathering and assessment) as part of the planning process.
• Ensure that assessments and plans are revised as necessary and whenever there are changes in the member’s life circumstances.
• Must fulfill QP status.
• Fully licensed by the NC governing board that regulates Human Services professions; OR a fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing, with a minimum of four (4) years of full-time experience serving the MH/DD/SA population.
• OR a graduate with a Master’s degree in a human service field and has one year of full-time, pre or post-graduate degree experience in the MH/DD/SA population, or a substance abuse professional with one year of full-time, pre or post-graduate degree supervised experience in alcoholism and drug abuse counseling.
• OR a graduate with a bachelor's degree in a human service field and has two years of full-time, pre or post-bachelor's degree experience with the MH/DD/SA population, or a substance abuse professional with two years of full-time, pre or post-bachelor's degree supervised experience in alcoholism and drug abuse counseling.
• OR a graduate with a bachelor's degree in a field other than human services and has four years of full-time, pre or post-bachelor's degree experience with the MH/DD/SA population, or a substance abuse professional with four years of full-time, pre or post-bachelor's degree supervised experience in alcoholism and drug abuse counseling.
• Additionally, for those serving members with LTSS needs, the care manager must meet the minimum QP qualifications defined above (A-D) and possess at least two (2) years of relevant LTSS and/or HCBS coordination, care delivery monitoring, and care management experience. This experience may overlap with the years required to attain Qualified Professional status.
• License and/or certification may be accepted as specified above.
• Must hold a valid driver’s license.
• Must reside within Trillium’s Catchment area in North Carolina.
• Typical working hours are from 8:30 am to 5:00 pm; flexible work schedules are available with management approval for certain roles.
