This is a fully remote position, open to applicants in Illinois.
• Oversee health and disease management programs within workplace and community environments in collaboration with Managed Care/ACO.
• Create initiatives for commercial insurance contracts.
• Maintain databases for biometric information.
• Track and review claims to monitor the return on investment for program efforts.
• Gather and analyze data related to employee wellness and benefits.
• Offer personalized health and disease management education.
• Work alongside interdisciplinary teams to assess, monitor, implement, and evaluate the health needs of employees.
• Coordinate resources centered on participants and promote independence in managing chronic diseases.
• Enroll qualified participants and deliver health and disease management to mitigate or eliminate high-risk behaviors.
• Organize and conduct onsite wellness seminars and training sessions.
• Develop and disseminate educational materials related to health and disease.
• Create and assess tailored care plans for participants.
• Design and execute health education programs for employees.
• Keep health education materials up to date in line with advancements in healthcare technology.
• Ensure the security, confidentiality, and integrity of databases.
• Assist in evaluating the success of current programs.
• Bachelor's degree in Nursing, Nutrition, or Dietetics.
• Valid Illinois RN License or Registered Dietitian credential.
• A minimum of 5 years of relevant technical experience.
• Familiarity with Joint Commission standards, CMS Core Measure requirements, and other industry best practices.
• Proficient in collecting and analyzing data related to employee wellness and benefits.
• Skilled in interacting with third-party payers, physicians, community organizations, and colleagues.
• Capable of providing personalized health and disease management education.
• Competent in assessing, monitoring, implementing, and evaluating employee health requirements.
• Adept at maintaining the security, confidentiality, and integrity of participant databases.
• Ability to accommodate participant scheduling needs.
