Hospice Triage RN, PT

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Conduct telephone triage for patients and families in hospice care.

• Evaluate patient conditions and decide on the appropriate next steps.

• Collaborate with on-call teams to ensure care and resources are coordinated effectively.

• Document communications and interventions with precision.

• Maintain a composed and professional demeanor while managing urgent calls.

• Deliver essential after-hours support and facilitate timely interventions for hospice patients and their families.

⛳️ Requirements

• Must possess an active multistate Registered Nurse (RN) license.

• Prior experience in hospice, palliative, or end-of-life care is strongly preferred.

• Comfortable using technology and electronic medical records (EMR) for documenting calls.

• Capable of typing documentation and notes in a fast-paced environment.

• Able to manage stress and multitask while receiving at least 5 calls per hour on weekdays and up to 8 per hour on weekends.

• Strong communication and critical thinking abilities.

• Must be able to work independently in a remote setting.

• Reliable attendance, effective communication, and stable internet connectivity during all scheduled shifts are essential.
