Hospice Triage RN, PT

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Deliver telephone triage for hospice patients and their families.

• Evaluate patient conditions and identify appropriate next steps.

• Collaborate with on-call teams to manage care and resources effectively.

• Meticulously document all communications and interventions.

• Maintain a composed and professional demeanor while addressing urgent calls.

⛳️ Requirements

• Valid multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly desirable.

• Proficient in technology and electronic medical records (EMR) for call documentation.

• Capable and comfortable typing documentation and notes in a fast-paced setting.

• Able to manage stress and multitask while receiving calls (minimum of 5 calls per hour on weekdays, and up to 8 per hour on weekends).

• Excellent communication and critical thinking skills.

• Ability to work independently in a remote setting.

• This position is remote and requires consistent attendance, effective communication, and reliable internet connectivity during all scheduled shifts to facilitate timely coordination of patient care.
