Hospice Triage RN, PT

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Deliver telephone triage services for hospice patients and their families.

• Evaluate patient conditions and identify suitable next steps.

• Work in collaboration with on-call teams to coordinate care and resources effectively.

• Ensure accurate documentation of all communications and interventions.

• Maintain a composed and professional demeanor while managing urgent calls.

⛳️ Requirements

• Possession of an active multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficiency with technology and electronic medical records (EMR) for documenting calls is essential.

• Ability to type documentation and notes swiftly in a fast-paced setting.

• Must effectively handle stress and multitask when receiving calls (minimum of 5 calls hourly on weekdays and up to 8 per hour on weekends).

• Strong communication and critical thinking abilities are required.

• Capacity to work independently in a remote setting.

• Consistent attendance, proactive communication, and reliable internet connection during all scheduled shifts are necessary to support timely patient care coordination.
