Home Care Account Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Assist in the onboarding process of new home care agencies and other partners by facilitating workflow setup, preparing documentation, and coordinating training.

• Provide ongoing onboarding assistance and manage handoff coordination alongside the Home Care and Channel Partnerships Manager.

• Act as the primary operational liaison for assigned home care partners and specific partnerships.

• Maintain consistent communication with partners to enhance service delivery and ensure effective referral execution.

• Address partner inquiries and requests; prioritize and escalate issues as necessary.

• Aid in relationship-building initiatives to enhance engagement, responsiveness, and alignment with Ceresti's expectations.

• Assist in resolving operational challenges such as scheduling delays, staffing shortages, missed visits, or documentation issues.

• Monitor and evaluate partner performance metrics, including referral response times, in-home assessments, respite fulfillment, documentation accuracy, and caregiver feedback.

• Ensure partners adhere to CMS GUIDE documentation and reporting standards.

⛳️ Requirements

• Bachelor’s degree in a relevant field or equivalent experience.

• 2–4 years of experience in home care, healthcare operations, account management, or partner support roles.

• Proven experience in supporting external partners, vendors, or clients within an operational setting.

• Understanding of home care workflows, including scheduling, staffing, and visit documentation.

• Excellent communication and relationship management abilities.

• Exceptional attention to detail and strong organizational skills; capable of managing multiple accounts and priorities simultaneously.

• Proficient in working with operational dashboards, KPIs, and performance reporting.
