Health Services Coordinator

This is a fully remote position, open to applicants in Virginia, +1 more state.

📋 Description

• Act as the main contact for patients who are newly referred.

• Perform outbound outreach to newly enrolled prospective patients to inform them about AMC Health's Remote Patient Monitoring programs, services, and advantages.

• Clearly and compassionately articulate program features, expectations, and value in a patient-centered way.

• Connect with patients over the phone to build rapport, address inquiries, and promote participation and adherence to monitoring programs.

• Execute necessary telephonic interventions, including Welcome Calls and Post-Physician Visit Updates.

• Accurately and thoroughly document patient information in the electronic health record (EHR).

• Work in collaboration with nurse care managers and the wider care team to achieve patient care objectives.

• Monitor hospitalizations and inform nursing staff upon patient discharge as needed.

• Coordinate patient assistance services as instructed, including referrals.

• Handle incoming inquiries from patients, clients, and providers while accurately assessing service needs.

• Adhere to all company policies, procedures, HIPAA regulations, and relevant federal and state laws.

⛳️ Requirements

• Minimum of six (6) months of experience in general Medical Assistant duties OR three (3) years of experience in a clinic or similar healthcare environment with direct patient interaction.

• Preferably completed a Medical Assisting program accredited by CAAHEP or ABHES.

• Familiarity with medical terminology and prior experience with electronic health record (EHR) systems.

• Strong technical skills, including the capability to assist patients remotely with health devices and resolve basic issues.

• Ability to type at least 50-60 words per minute while conversing with patients by phone.

• Current CNA certification or equivalent certification (e.g., Pharmacy Technician), in good standing.
