Health Coach – Contract

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver remote patient care using Revero’s app, engaging through text messaging, phone calls, and video conferencing.

• Educate and coach patients to facilitate the implementation of customized nutrition and care plans that promote sustainable lifestyle changes.

• Collaborate with Revero’s medical staff to address patient progress in a thorough, prompt, and effective manner; relay any medication modifications and physician recommendations to patients.

• Track and offer feedback on daily metrics such as symptoms, energy levels, cravings, hunger, and mood.

• Build rapport, connect, and collaborate with patients to ensure their personal success and sustainability.

• Review and update patients' medications, medical history, and lab results, while accurately documenting patient interactions in Revero’s portal.

⛳️ Requirements

• Bachelor’s degree in Nutrition, Dietetics, Kinesiology, Exercise Science, Nursing, Social Work, Psychology, or a related health field, or hold certifications as a Registered Dietitian, Registered Nurse, Registered Exercise Physiologist, or similar qualifications.

• Exceptional verbal and written communication capabilities.

• High emotional intelligence and empathy, coupled with a strong passion for building relationships with patients and a willingness to support others.

• Meticulous attention to detail and excellent organizational skills, demonstrating unwavering integrity.

• Proficiency in technology to effectively utilize diverse digital and virtual care platforms.

• At least 1 year of prior coaching experience.
