📋 Description

• Clinical Path Mastery: You take the initiative in managing care, ensuring that every patient adheres to an optimized, evidence-based plan, preventing complications before they necessitate emergency measures.

• Systemic Efficiency: You will pinpoint inefficiencies in the referral and authorization processes and eliminate them, ensuring that administrative obstacles never impede life-critical care.

• Resource Synthesis: You will design and implement a tailored network of community, clinical, and financial resources for each case, treating every patient's recovery as a high-stakes endeavor.

• Stakeholder Alignment: You serve as the definitive source of information among doctors, insurers, and families, guaranteeing 100% consensus on the care objectives.

⛳️ Requirements

• Clinical Foundation: You possess a professional degree (RN, BSN, LCSW, or MSW) and have experience working directly in acute care, home health, or complex care coordination.

• Navigated Complexity: You have a demonstrated history of managing high-acuity cases, making critical decisions under pressure.

• Mastered the Tools: You are adept with technology. You not only use EHRs but also utilize data and digital tools to streamline your workflow and monitor patient progress.

• Advocated & Won: You can provide examples of occasions where your intervention directly prevented a hospital readmission or successfully obtained a denied service for a patient.

• **Minimum Technical and Work Environment Requirements:**

• - **Internet Connection:**

• - Primary internet connection with a minimum speed of **15 Mbps**.

• - Backup internet connection with at least **10 Mbps**.

• - Backup connection must support work during a power outage.

• - **Primary Device:**

• - Desktop or laptop equipped with at least:
