This is a fully remote position, open to applicants in Illinois, +4 more states.
• Coordinate care for members through telephonic and/or field-based management.
• Educate and inform members about health care programs.
• Facilitate discussions with members regarding their adherence to personal health goals.
• Address inquiries and concerns from members.
• Assist clinical operations with activities involving providers and members.
• Conduct telephonic and/or field-based health screenings for members.
• Perform comprehensive health assessments and care planning that may require home or facility visits.
• Must be a Registered Nurse (RN) with at least 2 years of direct clinical experience in a clinical setting, or an unrestricted Licensed Professional Counselor (LPC, TX), or other professionals with 2 years of clinical practice experience.
• Hold a current, valid, and unrestricted license in the state of operation (or possess reciprocity).
• Additionally, have 3 years of experience in wellness or managed care, specifically in presenting clinical issues to members and physicians.
• Possess knowledge of the health and wellness marketplace and current employer trends.
• Demonstrate strong verbal and written communication skills.
• Have analytical experience, including the analysis of medical data.
• Show proficiency in PC applications such as Word, Excel, PowerPoint, as well as experience with databases and web-based applications.
• Maintain a current unrestricted driver’s license, along with transportation and applicable insurance.
