Geriatric Nurse Practitioner – Telehealth, Longevity Program

This is a fully remote position, open to applicants in Texas.

📋 Description

• Execute thorough initial longevity evaluations that encompass functional status, cognitive abilities, nutritional needs, mobility, and the risk of falls.

• Offer medical follow-up appointments for the management of chronic diseases and continued care.

• Carry out cognitive screenings and staging; refer for further evaluation when necessary.

• Prescribe and oversee GLP-1 therapy for qualified patients.

• Engage in advance care planning discussions.

• Record documentation in the EHR to facilitate accurate Medicare billing.

• Provide telehealth assessments and follow-up services to individuals aged 65 and older.

• Collaborate with the CMO, a board-certified geriatrician, receiving clinical training and support.

⛳️ Requirements

• Possession of an active, unrestricted NP license in Texas.

• Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or Family NP with a geriatric focus, training, or a demonstrated interest in aging.

• Active DEA registration.

• Proficient in Medicare documentation and E/M coding.

• Comfort with telehealth practices and EHR documentation.

• Access to reliable high-speed internet and a private, HIPAA-compliant workspace.

• Experience in conducting comprehensive geriatric assessments.

• Familiarity with cognitive screening tools (such as Mini-Cog and MoCA).

• Experience in prescribing GLP-1 medications.

• Multi-state licensure.
