Geriatric Nurse Practitioner – Telehealth, Longevity Program

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Conduct thorough initial assessments focused on longevity, including evaluations of functional status, cognitive abilities, nutritional needs, mobility, and fall risk.

• Provide follow-up medical visits aimed at chronic disease management and continuous care.

• Execute cognitive screenings and staging; refer for further evaluation when necessary.

• Prescribe and oversee GLP-1 therapy for eligible patients.

• Engage in advance care planning discussions.

• Record documentation in the EHR to ensure accurate Medicare billing.

⛳️ Requirements

• Current, unrestricted NP license in New Jersey.

• Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or Family NP with a focus on geriatrics, relevant training, or a demonstrated interest in aging.

• Valid DEA registration.

• Proficient in Medicare documentation and E/M coding.

• Comfortable with telehealth practices and EHR documentation.

• Access to reliable high-speed internet and a private, HIPAA-compliant workspace.

• Experience in conducting comprehensive geriatric assessments.

• Knowledge of cognitive screening tools (Mini-Cog, MoCA).

• Experience with GLP-1 prescribing.

• Possession of multi-state licensure.
