Geriatric Nurse Practitioner – Telehealth, Longevity Program

This is a fully remote position, open to applicants in Florida.

📋 Description

• Carry out thorough initial longevity evaluations that include assessing functional status, cognitive abilities, nutritional needs, mobility, and risk of falls.

• Offer medical follow-up appointments for managing chronic conditions and providing continuous care.

• Execute cognitive assessments and classifications; refer for additional evaluation as necessary.

• Prescribe and oversee GLP-1 therapy for eligible patients.

• Engage in advance care planning discussions.

• Record details in the EHR to ensure accurate Medicare billing.

• Collaborate with the Chief Medical Officer, a board-certified geriatrician, receiving both clinical training and support.

⛳️ Requirements

• Possession of an active, unrestricted NP license in Florida.

• Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or Family NP with a focus on geriatrics, along with relevant training or demonstrated interest in aging.

• Active DEA registration required.

• Proficiency in Medicare documentation and E/M coding.

• Comfort with telehealth modalities and EHR documentation practices.

• Access to reliable high-speed internet and a private workspace that complies with HIPAA regulations.

• Experience in comprehensive geriatric assessment is preferred (nice to have).

• Familiarity with cognitive screening tools such as Mini-Cog and MoCA is a plus (nice to have).

• Prior experience in GLP-1 prescribing is advantageous (nice to have).

• Multi-state licensure is beneficial (nice to have).
