Geriatric Nurse Practitioner – Telehealth, Longevity Program

This is a fully remote position, open to applicants in California.

📋 Description

• Conduct thorough initial assessments on longevity, evaluating functional status, cognition, nutrition, mobility, and fall risk.

• Provide follow-up medical visits focused on chronic disease management and continuous care.

• Perform cognitive screenings and staging; refer for additional assessments when necessary.

• Prescribe and oversee GLP-1 therapy for eligible patients.

• Engage in advance care planning discussions.

• Document in the EHR to ensure accurate billing for Medicare.

• Collaborate with the CMO, a board-certified geriatrician, receiving complete training and clinical support.

⛳️ Requirements

• Active, unrestricted Nurse Practitioner license in California.

• Board certification as an Adult-Gerontology Nurse Practitioner (AGNP/AGPCNP) or Family Nurse Practitioner with a focus on geriatrics, training, or a demonstrated interest in aging.

• Current DEA registration.

• Proficient in Medicare documentation and Evaluation and Management (E/M) coding.

• Comfortable with telehealth services and EHR documentation.

• Access to reliable high-speed internet and a private, HIPAA-compliant workspace.

• Experience in conducting comprehensive geriatric assessments.

• Knowledge of cognitive screening tools (Mini-Cog, MoCA).

• Experience in prescribing GLP-1 medications.

• Multi-state licensure preferred.
