Geriatric Nurse Practitioner – Telehealth

This is a fully remote position, open to applicants in New York.

📋 Description

• Conduct thorough initial assessments focusing on longevity, including evaluations of functional status, cognition, nutrition, mobility, and fall risk.

• Provide follow-up medical visits aimed at managing chronic diseases and ensuring continuous care.

• Execute cognitive screenings and stage assessments, escalating cases for further evaluation as necessary.

• Prescribe and oversee GLP-1 therapy for eligible patients.

• Engage in advance care planning discussions.

• Accurately document in the EHR to facilitate proper Medicare billing.

• Collaborate with the CMO, a certified geriatrician, who provides clinical training and support.

⛳️ Requirements

• Possession of an active, unrestricted NP license in New York.

• Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or as a Family NP with a focus on geriatrics, along with relevant training or a demonstrated interest in aging.

• Active DEA registration.

• Experience in Medicare documentation and E/M coding.

• Proficiency with telehealth services and EHR documentation.

• Access to reliable high-speed internet and a private, HIPAA-compliant workspace.

• Background in comprehensive geriatric assessment.

• Familiarity with cognitive screening tools such as Mini-Cog and MoCA.

• Experience in prescribing GLP-1 medications.

• Multi-state licensure is a plus.
