Geriatric Nurse Practitioner – Telehealth

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Conduct thorough initial longevity evaluations (60 min) encompassing functional status, cognition, nutrition, mobility, and fall risk.

• Provide ongoing medical follow-up visits for chronic disease management and continuous care.

• Execute cognitive screenings and staging; escalate for additional evaluation when necessary.

• Prescribe and oversee GLP-1 therapy for eligible patients.

• Engage in advance care planning discussions.

• Document in the EHR to facilitate accurate Medicare billing.

• Offer telehealth services to adults aged 65 and older.

• Collaborate with the CMO, a board-certified geriatrician.

⛳️ Requirements

• Active, unrestricted Nurse Practitioner license in Michigan.

• Board certification as an Adult-Gerontology Nurse Practitioner (AGNP/AGPCNP) or Family Nurse Practitioner with a focus on geriatrics, training, or demonstrated interest in aging.

• Active DEA registration.

• Proficiency in Medicare documentation and Evaluation and Management (E/M) coding.

• Comfort with telehealth practices and EHR documentation.

• Reliable high-speed internet connection and a private, HIPAA-compliant workspace.

• Experience in comprehensive geriatric assessments.

• Familiarity with cognitive screening tools (Mini-Cog, MoCA).

• Experience in prescribing GLP-1 medications.

• Multi-state licensure.
