FL Licensed Therapist, LMFT, LCSW, LPC

This is a fully remote position, open to applicants in Florida.

📋 Description

• Evaluate, plan, and execute personalized care strategies aimed at providing the most suitable and least restrictive level of care.

• Work collaboratively with members, families, and healthcare providers to create tailored care plans.

• Facilitate individual counseling sessions and group therapy for adolescents and adults.

• Recognize and initiate referrals for financial, psychosocial, community, and state support services.

• Advocate on behalf of members and families to ensure that patient needs and preferences are acknowledged and met.

• Apply authorized clinical criteria to evaluate and decide on the appropriate level of care.

• Maintain records of member assessments, care plans, and referrals.

• Meet established goals and Key Performance Indicators (KPIs).

• Familiarize yourself with the StrongWell model and take responsibility for outcome-focused care.

• Engage in interdisciplinary team meetings and utilization management discussions.

• Provide information to facilitate safe transitions of care.

• Encourage responsible and ethical management of company resources.

• Ensure punctuality and reliable attendance during work hours.

⛳️ Requirements

• Must possess an LCSW, LMFT, LPC, or LMHC license in Florida.

• Preferred experience working with substance abuse populations.

• Background in maternal and substance abuse care is a plus.

• Experience with both adults and adolescents is required.

• Knowledge of Medicare and Medicaid procedures is preferred.

• Previous experience in remote work is desirable.
