This is a fully remote position, open to applicants in Illinois.
• Conduct primarily in-person and telephonic behavioral health evaluations with members.
• Act as the main point of contact, delivering integrated, timely, and high-quality care coordination.
• Oversee member health issues and identify as well as resolve obstacles to effective care.
• Offer personalized care coordination, crisis intervention, peer support, and collaboration with medical and behavioral health professionals.
• Assist members with Serious Mental Illness (SMI), Serious Emotional Disturbance (SED), Substance Use Disorder (SUD), and those involved with the justice system across children, adolescents, and adults.
• Engage members within their communities and perform face-to-face visits.
• Complete necessary assessments, including the Comprehensive Risk Assessment (HRA).
• Coordinate behavioral health and medical services, provider engagement, and adherence to treatment plans.
• Enhance member health literacy and address health-related social needs.
• Function as a member's interdisciplinary care team, managing care planning, transitions, and service delivery.
• Facilitate interdisciplinary care team meetings and communication among providers, Service Coordinators, and Care Management Extenders.
• Support transitions of care.
• Conduct biannual and quarterly in-person visits for monitoring and intervention.
• Collaborate with internal departments, providers, and community-based organizations.
• Adhere to processes and procedures to meet IDHS, CMS, and NCQA requirements.
• Perform other assigned duties.
• Must live in Cook, DuPage, DeKalb, or Kane County, Illinois.
• Active Illinois license as an LCSW, LMFT, or LCPC; no supervisees or provisional licenses permitted.
• A minimum of 2 years of post-degree clinical experience in a behavioral health environment.
• Experience in case management with complex SMI, SUD, and SED populations.
• Ability to travel to regional facilities and homes for in-person assessments.
• Valid driver's license, car insurance, and dependable transportation required.
• Capability to work independently while seeking support when necessary.
• Must comply with IDHS, CMS, and NCQA regulatory standards.
• Up to 75% travel required for collaboration, in-person meetings, and field interactions.
• Work schedule is Monday–Friday, 8:00 a.m.–5:00 p.m. Central Standard Time.
• TB screening must be completed if selected.
• Must maintain personal vehicle liability insurance that meets state minimums or specified higher limits.
• Home internet connection with a minimum of 25 Mbps download and 10 Mbps upload speed is necessary.
• A dedicated workspace free from ongoing interruptions to safeguard PHI/HIPAA information is required.
• Preferred: CCM certification.
• Preferred: 3+ years of in-home assessment and care coordination experience.
• Preferred: experience with Medicare, Medicaid, and dual-eligible populations.
• Preferred: field case management experience.
• Preferred: knowledge of community health and social service agencies and resources.
• Preferred: previous experience in managed care.
• Preferred: bilingual proficiency.
