This is a fully remote position, open to applicants in Montana.
• Operate in a continuous coordinator capacity under the guidance of a fully certified Family Support Specialist (FSS).
• Aid in determining eligibility, which encompasses referral, intake, and eligibility evaluation tasks.
• Perform family assessments, which include interviewing family members.
• Gather information regarding the child’s development, including direct observations of the child.
• Organize evaluations and assessments.
• Ensure families are aware of their rights, options, advocacy services, and their involvement in the EI process.
• Support parents of eligible children in planning, conducting, developing, implementing, and reviewing an Individualized Family Service Plan (IFSP).
• Collect the family's insurance or Medicaid details to assist in coordinating benefits and services.
• Keep precise, complete, and timely electronic and physical file records.
• Oversee the provision of services to guarantee that they are delivered punctually and in alignment with the IFSP.
• Coordinate and supervise the provision of available services.
• Communicate and coordinate with medical and health service providers.
• Facilitate the creation of a transition plan for entry into, within, and exiting from the EI system, including transitions to Part B special education, preschool, or other relevant community services.
• Meet weekly with the supervisor for direct, systematic supervision of services through regular (minimum requirement) instruction, review, and feedback from the fully certified FSS.
• Perform additional tasks as assigned by the supervisor.
• A valid Driver's License and auto insurance are required.
• The minimum educational requirement is a high school diploma or its equivalent.
• Completion of on-the-job training and certification by the State of Montana within two years of hire is mandatory.
