This is a fully remote position, open to applicants in Colorado.
• Perform targeted assessments and virtual consultations for patients with hypertension, hyperlipidemia, coronary artery disease, and diabetes.
• Develop diagnoses and personalized treatment strategies, which include medication management, lifestyle changes, and monitoring guidelines.
• Oversee prescription refill inboxes in accordance with established protocols and guidelines.
• Evaluate, prioritize, and follow up on laboratory and diagnostic findings through centralized results management processes.
• Utilize live portal chat and AI-enhanced clinical tools for prescribing, inbox triage, medication renewals, result management, and patient communications.
• Recognize risk factors, comorbidities, and contraindications to treatment.
• Advise patients on adherence to medication and possible side effects.
• Collaborate with primary care providers, specialists, pharmacists, clinical nursing staff, social workers, family members, and caregivers.
• Use the electronic health information system during visits and provide feedback on its interface.
• Ensure accurate maintenance of patient medical records.
• Engage in quality improvement projects and continuous professional development.
• Comply with legal, ethical, privacy, telehealth, and industry standards.
• Advanced Practice Provider (NP or PA) qualification.
• A minimum of three years of experience in outpatient clinical practice.
• At least two years of experience focusing primarily on primary care or a subspecialty with a strong emphasis on hypertension and lipid management.
• For Nurse Practitioners: must hold an active and in-good-standing RN and APRN license.
• For Physician Assistants: must possess an active and in-good-standing Physician Assistant license.
• Licensure and certification for the role must be in good standing.
• Capability to obtain licensure in the required states within 90 days of the hiring date.
• Comprehensive knowledge of managing cardiometabolic conditions, including hypertension, hyperlipidemia, coronary artery disease, and diabetes.
• Experience in medication management and lifestyle modification.
• Familiarity with STARS/HEDIS measures, coding clinical comorbidities, and identifying gaps in clinical care.
• Proficiency in English and Spanish is preferred for this role.
• Ability to adhere to laws, regulations, industry standards, patient privacy, and telehealth regulations.
