This is a fully remote position, open to applicants in Arizona.
• Perform comprehensive and targeted virtual assessments of patients diagnosed with hypertension, hyperlipidemia, coronary artery disease, and diabetes.
• Develop diagnoses and personalized treatment strategies, which encompass medication management, lifestyle changes, and monitoring suggestions.
• Oversee a prescription refill inbox in alignment with established protocols and standards.
• Engage in centralized results management, including the review, triage, and follow-up of lab and diagnostic findings.
• Utilize live portal chat and AI-enhanced clinical tools for prescribing, inbox triage, medication renewals, results management, and patient communication.
• Recognize and assess risk factors, comorbidities, and contraindications for treatment.
• Advise patients on medication adherence and potential side effects.
• Collaborate effectively with PCPs, specialists, pharmacy, clinical nursing, social work, family members, and caregivers.
• Employ the electronic health information system during visits and provide feedback on the interface.
• Ensure accurate maintenance of patient medical records in accordance with legal and ethical standards.
• Engage in quality improvement initiatives and ongoing professional development activities.
• Comply with laws, regulations, industry standards, patient privacy requirements, and telehealth regulations.
• Advanced Practice Provider (NP or PA) qualifications.
• A minimum of three years of experience in outpatient clinical practice.
• At least two years of experience focused primarily on primary care or a subspecialty with a strong emphasis on hypertension and lipid management.
• Experience managing hypertension, hyperlipidemia, and both primary and secondary prevention of atherosclerotic cardiovascular disease is preferred.
• For Nurse Practitioners: possession of active and in-good-standing RN and APRN licenses.
• For Physician Assistants: possession of an active and in-good-standing Physician Assistant license.
• Required role licensure and certification must be in good standing.
• Capability to obtain licensure in requested states within 90 days of the hire date.
• Willingness to secure licenses in additional states as necessary.
• Comprehensive understanding of cardiometabolic condition management, covering disease processes, treatment options, medication management, and lifestyle modifications.
• Familiarity with STARS/HEDIS measures, coding clinical comorbidities, and identifying clinical care gaps.
• Prior virtual care experience is preferred.
• Proficiency in both English and Spanish is advantageous.
