This is a fully remote position, open to applicants in Utah.
• Facilitate the completion of CHE in nephrology offices, dialysis clinics, or through telehealth; collaborate with Nurse Practitioners and clinics to prepare for face-to-face appointments.
• Conduct outbound calls to patients, caregivers, and clinics for scheduling CHEs, sending reminders, and rescheduling; arrange telehealth CHEs for patients who cannot attend in person.
• Welcome new patients into the program and gather any new concerns from existing patients; escalate issues to the RN or Nurse Practitioner when necessary.
• Maintain precise documentation and relay patient updates to the care team.
• Exhibit outstanding interpersonal skills to foster trust, encourage patient participation, and address barriers to engagement.
• Organize outreach initiatives based on urgency and importance to maximize patient impact.
• Coordinate patient care by ensuring access to eligible resources and programs, and assist with referrals and provider appointments.
• Carry out non-clinical screenings and pathways under the Manager's guidance; escalate to the Integrated Care Nurse as required.
• Support the clinical team by helping to move patients through the continuum of care by completing assigned tasks.
• Identify resources available for members and work with Integrated Care Nurses, dialysis clinics, health plans, and other stakeholders to assist members in enrolling in programs.
• Communicate promptly and professionally with patients.
• Keep accurate, complete, and confidential documentation.
• Demonstrate exceptional two-way communication skills, both oral and written.
• Operate in alignment with DaVita’s mission and values to provide high-quality patient care.
• High School Diploma or GED is required; an Associate’s or Bachelor’s Degree is preferred.
• Previous IKC experience is required.
• A minimum of 2 years of professional experience in customer service, patient-facing, or telephonic outreach roles is preferred.
• Prior experience in the healthcare field is preferred.
• Intermediate proficiency in Microsoft Word, Excel, PowerPoint, and Outlook is necessary.
• Strong verbal and written communication abilities, active listening skills, and motivational engagement techniques are essential.
• Highly organized with excellent time management skills and follow-through.
• Capability to work both independently and collaboratively with care team members.
