ECM Territory Manager – Nor Cal

This is a fully remote position, open to applicants in California.

📋 Description

• Achieve performance metrics, objectives, and deadlines for the team while inspiring them to surpass these targets.

• Oversee the daily operations within their territory, which includes conducting individual performance evaluations and caseload assessments, as well as monitoring individual productivity, growth, member engagement, quality of care, and graduation rates.

• Collaborate with community-based organizations, health plans, and other strategic partners.

• Deliver continuous virtual and field-based coaching and development for the field team.

• Provide resources related to community-based opportunities in conjunction with the Community Impact Partnership Program (CIPP) Director.

• Guide Community Health Navigators in formulating Specific, Measurable, Achievable, Relevant, Time-based (SMART) goals with feasible interventions and assist them in overcoming barriers and objections faced by members.

• Address issues raised by Community Health Navigators in a proactive and innovative manner.

• Assist in program expansion, staffing requirements, and operational improvements.

• Recruit, manage, and mentor staff members.

• Oversee Profit and Loss for the designated territory.

• Ensure the prompt review and approval of payroll, expenses, and other compliance obligations within the territory.

⛳️ Requirements

• A minimum of 3-4 years of experience in managing teams within health, social, or community services.

• High School Diploma or its equivalent.

• Proficient analytical skills with a solid understanding and comfort level with data, dashboards, and reporting tools.

• Capability to manage a Profit and Loss statement.

• Ability to adapt to continuous change and excel under pressure in a fast-paced and demanding environment.

• Strong problem-solving skills, critical thinking abilities, and effective collaboration skills.

• A curious and tenacious disposition.

• Knowledge of patient-centered care and holistic care models.
