Director, Clinical Operations – Transitions of Care

This is a fully remote position, open to applicants in New York.

📋 Description

• Oversee and enhance the 30-day transition process from admission and discharge alerts through patient visits, escalations, and follow-ups.

• Expand medication reconciliation, durable medical equipment (DME), home health services, primary care provider (PCP) and specialist scheduling, transportation, and caregiver support.

• Mentor a team of distributed Nurse Practitioners (NPs), Registered Nurses (RNs), and Medical Assistants (MAs).

• Establish protocols, maintain documentation, conduct quality checks, and generate reports.

• Collaborate with product, engineering, and health plans to enhance readmission rates, improve PCR and TRC/HEDIS Stars measures, and optimize program finances.

• Report directly to the Chief Clinical Officer.

• Collaborate with an established team.

⛳️ Requirements

• Proven leadership experience in transitions of care, post-acute services, or care management operations.

• Demonstrated ability to scale or significantly enhance a post-discharge program.

• In-depth knowledge of post-acute interventions.

• Experience in training clinical teams.

• Familiarity with performance metrics for health plan populations.

• Ability to create workflows in partnership with clinicians and cross-functional teams within a growth-oriented environment.

• Experience in launching a program from the ground up is advantageous, though not mandatory.

• A clinical license is a beneficial asset.

• Background in home-based care, value-based arrangements, or ADT/HIE workflows is a plus.
