This is a fully remote position, open to applicants in United States.
• Deliver innovative leadership and strategic guidance for care coordination and service delivery teams that support healthcare programs.
• Establish and implement strategic initiatives aimed at enhancing care coordination and scheduling services.
• Collaborate with senior executives to influence program strategies and respond to emerging trends in healthcare delivery and payer programs.
• Manage scalable workflows, operational policies, and resource allocation.
• Drive cross-functional initiatives to tackle systemic challenges, implement process enhancements, and ensure compliance.
• Utilize data analytics and key performance indicators (KPIs) to assess program effectiveness and inform evidence-based decisions.
• Execute quality improvement initiatives to improve patient and partner outcomes.
• Share insights and recommendations during executive discussions.
• Oversee the operational strategy and implementation of the ED Transitions model across ED and risk-bearing entity partnerships.
• Create performance management frameworks, productivity benchmarks, and operational dashboards.
• Guide scalability initiatives for ED Transitions, including workforce planning, operating model development, governance, implementations, and cross-functional collaboration.
• Integrate technology-driven workflows, analytics, AI-assisted tools, and operational best practices.
• Undertake additional job responsibilities as assigned.
• A minimum of 8-10 years of relevant work experience.
• Bachelor's degree or equivalent professional experience.
• At least 4-5 years of experience in people management.
• Willingness to travel up to 30% based on business requirements.
• Availability to work additional or irregular hours as necessary.
• Compliance with applicable security policies and procedures to protect company and client information.
• Ability to sit and view a computer screen for prolonged periods.
• Candidates for U.S.-based positions must possess legal authorization to work in the United States.
