This is a fully remote position, open to applicants in Vermont.
• Oversee the creation of an implementation-ready program model for a Coordinated Specialty Care (CSC) initiative in Vermont, collaborating with content experts, service providers, individuals who have experienced psychosis along with their families, and other stakeholders as an initial step toward statewide rollout.
• Manage the development of operational supports for program delivery, which includes program policies and procedures, referral processes, admission criteria, discharge protocols, job descriptions, and structures for advisory boards.
• Facilitate stakeholder engagement activities and aid in the creation and execution of a stakeholder engagement and advisory board plan.
• Recruit and gather stakeholders, including national content experts, individuals who have experienced psychosis, their families, and others as needed for guidance and input on program implementation.
• Assist in engaging community leaders and organizations to advocate for and support the establishment of a future CSC team.
• Collaborate with community partners to form essential partnerships for program implementation, including investigating MOUs or subrecipient agreements.
• Partner with Pathways Vermont Training Institute, subject matter experts, and established national models to develop a thorough Coordinated Specialty Care Training Curriculum that meets best practice requirements.
• Create and present outreach and educational materials while engaging referral partners.
• Participate in messaging that aligns with values and best practices, providing opportunities for community education and involvement.
• Oversee the development and successful completion of all contract deliverables, including preparing an implementation plan report outlining the proposed service delivery model, staffing structure, governance, recommended pilot site, referral pathways, and how stakeholder feedback is incorporated into the final model.
• Draft an implementation plan blueprint for the state detailing the operational approach for launching a single-site CSC pilot team.
• Prepare a Coordinated Specialty Care training plan and curriculum.
• Develop a policies and procedures manual for Coordinated Specialty Care teams.
• Collaborate with Pathways leadership to ensure the preparation and completion of a financing and billing recommendations report for the state.
• Work with Pathways leadership to deliver the aforementioned materials to the state in their entirety and on schedule.
• Create and maintain project timelines and work plans.
• Monitor milestones, deliverables, and contract requirements.
• Organize meetings, agendas, and follow-ups.
• Identify project risks and escalate issues to agency leadership as necessary.
• Coordinate project consultants, stakeholders, and subject matter experts.
• Manage internal communication pertinent to the CSC project.
• Perform other duties as assigned.
• Attendance at the annual All Staff and Programmatic Retreats is a requirement of employment with Pathways Vermont, unless exempted by the employee’s manager/HR. These retreats occur on workdays during standard working hours and are fully compensated.
• Minimum of 3 years experience in project management and/or program implementation.
• Background in administration, database management, and proficiency in computer programs (including Microsoft Office Suite, Google Apps, Electronic Medical Records, Internet browsers, etc.).
• Ability to thrive in a fast-paced environment, demonstrating strong problem-solving skills, work ethic, and computer proficiency.
• Flexible working hours are required.
• A valid driver’s license, registration, insurance, and access to a vehicle are necessary.
• Occasional out-of-state travel may be required for evaluating CSC programs.
• Exceptional verbal and written communication skills.
