Complex Case Manager, RN – Specialty Pediatrics

This is a fully remote position, open to applicants in Louisiana, +4 more states.

📋 Description

• Oversee a panel of members with diverse health management requirements

• Evaluate health management needs and identify suitable clinical interventions

• Create care plans that include both short-term and long-term objectives

• Track quality outcomes related to case management

• Perform outreach to members participating in case management

• Work in collaboration with multi-disciplinary teams to ensure coordinated care

⛳️ Requirements

• High School Diploma or GED is mandatory

• A minimum of 7 years of experience in clinical, case management, or health insurance is required

• Advanced training in cognitive behavioral therapy (CBT), motivational interviewing, or dialectical behavior therapy (DBT) is preferred

• Current RN licensure in PA or multi-state licensure through eNLC is preferred

• Certification in Case Management is preferred

• Proficiency in MS Excel is essential
