Complex Case Manager RN – Oncology

This is a fully remote position, open to applicants in Louisiana, +4 more states.

📋 Description

• Oversee a designated panel of members

• Develop care plans tailored to the identified needs of members

• Achieve the targeted percentage of patient goal attainment

• Track, enhance, and sustain quality outcomes for the designated panel of members

⛳️ Requirements

• High School Diploma or GED

• A minimum of 7 years in a combination of clinical, case management, and/or disease or condition management experience

• Current RN licensure in the State of Pennsylvania or multi-state licensure through the enhanced Nurse Licensure Compact (eNLC), or in West Virginia, Delaware, or New York

• Certification in Case Management is preferred
