This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Engage in thorough System of Care planning, execution, coordination, and training pertaining to essential core functions within the operational Region.

• Offer education and support to members and their families to help them understand and exercise their rights and responsibilities.

• Collaborate as part of a multidisciplinary care team to apply person-centered planning techniques to collect information, allowing for a better understanding of the members and families/legally responsible persons to effectively meet individual needs.

• Aid in the completion of discovery activities (information gathering and assessment) as part of the planning process.

• Support members/legally responsible persons in selecting service providers, ensuring an objective approach throughout the process.

• Assist the member/family in leading the planning process/plan development, as much as desired by the member.


⛳️ Requirements

• High School Diploma or GED.

• At least four (4) years of experience as a primary caregiver for a child or youth receiving Medicaid or state-funded BH services.

• A valid driver’s license is required.

• Residency in one of the following North Carolina counties: Anson, Montgomery, Moore, or Richmond.

• Ability to travel within the catchment area as needed.

• Two (2) years of paid experience working directly with individuals with BH/MH or IDD conditions is preferred.

• Paid experience relevant to the counties where they are applying is preferred.

• Certification as a Peer Support Specialist is preferred.
