Community Resource Coordinator II

This is a fully remote position, open to applicants in Kansas.

📋 Description

• Facilitates community engagement initiatives by linking members to available community resources.

• Supplies members with information on recognized community resources while supporting the care team.

• Aids members in connecting with established community and care resources.

• Enhances understanding of care/services and manages all correspondence related to members.

• Records and updates community resources to uphold practice standards.

• Assists the clinical team and carries out non-clinical general health evaluations.

• Engages in both telephonic and/or in-person outreach to connect with individuals and families.

• Executes other assigned tasks as needed.

• Possession of a High School diploma or GED is required.

• A minimum of 1 – 2 years of relevant experience is necessary.
