Community Liaison, Western NC

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Educate members, recipients, and stakeholder entities regarding Transitions to Community Living (TCL), including eligibility criteria and the RSVP process.

• Inform providers and both internal and external stakeholders on recognizing and reporting obstacles.

• Assist in removing TCL barriers and develop or offer training opportunities.

• Collaborate with referral entities to evaluate member/recipient needs and inform stakeholders about suitable services and levels of care.

• Identify service deficiencies and take action to ensure proper care and coordinated services.

• Coordinate services across the LME/MCO system and other networks, including primary care.

• Provide education and support to local hospitals and other stakeholders regarding RSVP.

• Offer potential members/recipients insights into the TCL process during community outreach efforts.

• Educate facility stakeholders when individuals do not qualify for TCL eligibility and direct them to community resources.

• Carry out responsibilities for Diversion, In-Reach, Transition Process, and Post-Transition.

• Work in conjunction with the TCL team and community partners to assess needs and deliver outreach and education.

• Act as a voting member of the Vaya Local Barriers Committee.

• Organize, manage, and facilitate community stakeholder meetings.

• Lead and coordinate quarterly ACT/CST Collaborative Meetings.

• Gather data related to TCL and collaborate with teams to address identified needs.

• Accurately document contacts and interventions in administrative health records.

• Support monthly reporting obligations for the Department of Justice Settlement.

• Operate independently within the community, receiving monthly formal supervision and informal supervision as needed.

⛳️ Requirements

• A minimum of a bachelor's degree in public service administration, psychology, sociology, social work, or a related human service field, along with four years of directly related human services program experience; OR a four-year college degree with four years of administrative or consultative human service program experience; OR a master's degree in a pertinent human services discipline accompanied by two years of supervised clinical experience; or a suitable combination of education and experience.

• Relevant experience must include case management, assessment and referral, supportive counseling, intervention, psychosocial therapy, and treatment planning.

• Experience working with children, adolescents, and adults with MHSU and IDD diagnoses.

• Operational knowledge of funding sources and access to existing funds, or the capability to formulate or present specialized funding requests.

• Comprehensive understanding of TCL requirements, waiver and contractual obligations, and the TCLD database.

• Familiarity with the current Diagnostic and Statistical Manual of Mental Disorders.

• Awareness of the MHSU/IDD service array offered through LME/MCO provider networks.

• Knowledge of Medicaid eligibility determinations.

• Ability to independently identify, implement, and evaluate system and organizational priorities related to TCL initiatives.

• Strong verbal and written communication skills to convey information clearly and concisely.

• Capacity to make prompt independent decisions based on relevant facts and established protocols.

• Skills in problem-solving, negotiation, and conflict resolution.

• Ability to organize multiple tasks and priorities while managing an assigned caseload within deadlines.

• Proficiency in Microsoft Office applications, including Word, Excel, Outlook, and Forms, as well as Vaya Information Systems.

• Must reside in North Carolina or within 40 miles of the North Carolina border.

• Willingness to travel within the community and work in various locations.

• Close visual acuity, ability to walk, crouch, reach, talk, hear, perform repetitive hand/wrist/finger motions, and lift up to 10 pounds.
