Community Health Worker – Carceral Transitions

This is a fully remote position, open to applicants in Washington.

📋 Description

• Facilitate a successful transition from the correctional environment to the community.

• Recognize obstacles to healthcare and social services, connecting members to resources to enhance their health.

• Collaborate with the Carceral Transitions team, facility personnel, and the community to ensure effective re-entry for the member.

• Enhance individual and community capacity by increasing health knowledge and self-reliance through outreach, community education, referrals, peer support, and advocacy.

• Advocate for and coordinate resources necessary to assist justice-involved members in achieving optimum functional levels during their transition and provide short-term care coordination.

• Communicate the healthcare needs of the members to the correctional facility and transmit medical records when necessary.

• Identify Health Related Social Needs and link members with suitable community resources.

• Conduct meetings with members via phone or video conferencing at city and county jails, or the Department of Corrections.

• Arrange medical, dental, and/or vision appointments in line with the member's release date, including transportation to appointments if needed.

• Provide verbal and/or written updates regarding the member's progress and needs to the appropriate case managers, providers, legal mandates, or other affiliates involved in the care plan as necessary to support the program and its members.

• Ensure case documentation adheres to established policies and procedures.

• Conduct Health Risk Assessments with members.

⛳️ Requirements

• Possess a minimum of three (3) years of relevant experience delivering medical, mental health, or substance abuse-focused services to individuals with chronic medical conditions and/or severe and persistent mental illness in roles such as Community Health Worker, Patient Navigator, Healthcare Navigator, or similar positions.

• Be comfortable working with justice-involved individuals, at-risk populations, and diverse communities.

• Have experience utilizing health plan care management systems or electronic medical record systems.

• Hold certification as a Peer Specialist if you do not meet the minimum experience and educational criteria.

• Have experience collaborating with local, state, and federal resources for individuals with behavioral and/or physical health needs.
