Community Health Worker

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Collaborate with patients, their families, primary care teams, and community organizations to enhance access to healthcare.

• Tackle social determinants affecting health.

• Assist in self-management of chronic diseases.

• Link patients to medical, behavioral health, and community resources.

• Offer navigation through the health system and coordinate resources.

• Engage in outreach and educational initiatives.

• Reinforce established care plans.

• Deliver culturally sensitive coaching.

• Act as a reliable frontline public health representative.

• Clinical assessment, diagnosis, treatment, clinical case management, or services that require professional licensing are not provided.

⛳️ Requirements

• High School Diploma or GED is required.

• Relevant experience in healthcare, public health, community services, human services, care coordination, behavioral health, or social services.

• Familiarity with local community resources.

• Background in Medicaid, rural populations, behavioral health, chronic diseases, maternal-child health, or underserved communities is beneficial.

• Completion of, or a willingness to undertake, a Michigan-recognized Community Health Worker (CHW) training program.

• MiCHWA CHW certification or eligibility is strongly preferred.

• Candidates must reside in the East Region of Northern Michigan, particularly near Indian River, Grayling, Gaylord, or Mio.

• Proof of vaccination or lab-confirmed immunity for Measles, Mumps, Rubella, and Varicella is mandatory.

• Must obtain a flu vaccine during the flu season in the year of hire and annually thereafter, or present an approved medical or religious exemption.

• Bilingual abilities are preferred where applicable.
