Community Health Outreach Specialist

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Conduct initial and ongoing outreach to high-risk patients in person to educate them about the Monogram Health program and ensure they are utilizing its benefits.

• Articulate the advantages of the Monogram Health program to eligible members and address any objections they may have.

• Significant daily travel is required, as most of the day will be spent visiting community members in their homes and connecting them with care providers.

• Facilitate warm handoffs to the designated Monogram Health clinicians and customer support when a member expresses interest in the program, ensuring the initial enrollment appointment is scheduled and completed successfully.

⛳️ Requirements

• A minimum of 2 years’ experience in healthcare, sales, outreach, engagement, or enrollment campaigns.

• Proficiency in Salesforce and Microsoft Office is required.

• At least 2 years of B2B sales experience is required.

• A Bachelor's Degree is preferred, though not mandatory.

• A valid driver’s license is necessary.
