This is a fully remote position, open to applicants in Florida.
• Assess all incoming home health orders for clinical suitability.
• Identify the appropriate disciplines, frequencies, and types of visits according to agency protocols and referral requirements.
• Ensure adherence to CMS guidelines, Medicare Conditions of Participation (COPs), and internal agency standards.
• Accurately and efficiently take and document verbal orders from physicians and referral sources.
• Conduct patient welcome calls to facilitate a seamless and positive onboarding experience.
• Address and resolve patient needs or concerns during intake discussions.
• Maintain a warm, empathetic, and professional demeanor on the phone.
• Record intake and order-related activities in the EMR (WellSky or similar).
• Verify that physician orders, care plans, and necessary documentation are complete, accurate, and correctly uploaded.
• Stay informed about home health regulatory requirements, including Medicare, CMS, and state guidelines.
• Adhere to daily reporting and tracking protocols.
• Collaborate with Intake, Care Coordination, and Branch leadership to ensure timely initiation of patient care.
• Report clinical concerns or regulatory issues to the Clinical Review Manager.
• Assist branch and central teams with troubleshooting and providing guidance on referral-related matters.
• Current LPN or RN with relevant experience in home health intake or clinical review.
• Minimum of 2 years of clinical experience in home health, intake, or care coordination.
• Familiarity with Medicare COPs, CMS guidelines, and general home health regulations.
• Strong critical thinking and clinical decision-making abilities.
• Excellent communication and telephone skills; capable of building trust with patients, referral sources, and colleagues.
• Proficient in EMR systems (WellSky preferred) and Microsoft Office Suite (Outlook, Teams, Excel).
• Ability to thrive in a fast-paced, high-volume environment while maintaining accuracy and composure.
• Skilled in EMR data entry (WellSky preferred).
• Comfortable using Microsoft Outlook, Teams, and internal reporting tools.
• Availability to work two weekends per month, Saturday and Sunday, from 8:00 AM to 6:30 PM Eastern Time Zone.
