📋 Description

• Deliver medication counseling and support to members to reduce treatment gaps.

• Evaluate medication lists to identify and resolve barriers to effective medication therapy.

• Act as the member's Pharmacy Navigator, promoting medication adherence and preventing adverse medication-related events.

• Manage the process of obtaining refills and coordinating new prescriptions and appointments.

• Assist members facing financial difficulties in gaining access to medications.

• Synchronize refills to minimize the frequency of pharmacy visits.

• Document the care provided and relay pertinent information to other healthcare providers.

• Collaborate on medication treatment plans and Medication Therapy Management (MTM) interventions with Healthfirst Pharmacists and both internal and external partners.

• Partner with local pharmacies to oversee and communicate member medication adherence.

• Conduct patient health assessments that include prescriptions, over-the-counter medications, and homeopathic/herbal therapies.

• Manage communications with members and providers via phone; in-person interactions may be necessary in certain circumstances.

• Undertake additional responsibilities as required.

⛳️ Requirements

• High School Diploma or GED from an accredited institution.

• Professional experience that necessitates clear, concise, and grammatically correct written and verbal communication.

• Proven experience in communicating with patients, providers, and pharmacists regarding medications and related concerns.

• Must be either a nationally certified Pharmacy Technician in good standing (e.g., PTCB, NHA) or a registered or licensed Pharmacy Technician in good standing.

• Bilingual abilities are preferred.

• Medication Therapy Management Certificate is preferred.
