This is a fully remote position, open to applicants in Washington.
β’ Deliver comprehensive medication management, encompassing reconciliation, dose optimization, adherence support, and drug-level assessments in accordance with evidence-based guidelines.
β’ Collaborate with physicians, specialists, and interdisciplinary teams to create personalized care plans under a Collaborative Practice Agreement (CPA).
β’ Assess the effectiveness and safety of therapies, pinpoint opportunities for optimization, and take action as necessary.
β’ Contribute to quality improvement and Risk Evaluation and Mitigation Strategies (REMS) initiatives.
β’ Assist colleagues with complex prior authorization processes.
β’ Enhance workflows for improved efficiency.
β’ Provide education to patients regarding their medications.
β’ Engage in ongoing clinical development to stay current in the field.
β’ Work directly with care teams through virtual patient assessments.
β’ Address and resolve medication-related issues.
β’ Possess a Doctor of Pharmacy (PharmD) degree.
β’ Must hold an active pharmacist license in Washington.
β’ Completion of a PGY2 Ambulatory Care Residency, or a PGY1 Residency combined with at least one year of ambulatory practice under a Collaborative Practice Agreement (CPA), or a minimum of three years of ambulatory practice focusing on disease state management.
β’ Exceptional communication, organizational, and technological skills.
β’ Display empathy, integrity, and a team-oriented approach.
β’ Strong computer proficiency is required, including expertise in Microsoft Office applications (Outlook, Teams, Word, Excel, and PowerPoint).
β’ Competence in navigating the internet and various web-based platforms.
β’ Data entry and computer skills will be assessed during the pre-employment evaluation process.
β’ Must meet eligibility requirements to work as a licensed pharmacist in Washington.
