This is a fully remote position, open to applicants in United States.
• Deliver Medication Therapy Management services, encompassing Comprehensive Medication Reviews, Targeted Medication Reviews, adherence interventions, cost-effective initiatives, and other clinical interventions related to medication.
• Assess medication therapy to pinpoint and resolve safety issues, drug interactions, therapeutic duplications, therapy gaps, adherence obstacles, and opportunities for treatment optimization.
• Educate and counsel patients on medication therapy and adherence practices.
• Relay clinical recommendations and interventions to prescribers and members of the healthcare team.
• Engage in patient and provider outreach to facilitate medication management and address medication-related issues.
• Offer pharmacogenomic consultations, adherence and quality initiatives, transitions of care, and other designated clinical pharmacy services.
• Record clinical reviews, interventions, recommendations, outreach efforts, and outcomes in relevant systems.
• Collaborate with pharmacy technicians and interdisciplinary team members.
• Uphold clinical, quality, productivity, and documentation standards.
• Adhere to clinical protocols, client specifications, policies, procedures, and federal and state regulations.
• Take part in training, quality improvement activities, and the initiation of new clinical programs.
• Execute other responsibilities as assigned by the Director of Clinical Services, Manager, and Supervisor.
• Graduate from an accredited college of pharmacy with a Bachelor of Science in Pharmacy or a Doctor of Pharmacy (PharmD) degree.
• Possess an active pharmacist license in good standing.
• Capability to obtain and maintain licensure across multiple states as necessary.
• Preferred clinical pharmacy experience, including MTM, ambulatory care, population health, telepharmacy, medication adherence, pharmacogenomics, or other patient-centric pharmacy services.
• Strong clinical acumen and the ability to evaluate medication therapy and identify suitable interventions.
• Exceptional verbal and written communication skills.
• Strong organizational, time-management, critical-thinking, and problem-solving abilities.
• Capacity to work independently in a remote setting while engaging with a multidisciplinary team.
• Ability to quickly learn and navigate multiple clinical programs and adjust to changing client and organizational requirements.
• Proficient with computer systems, electronic health records, clinical documentation platforms, pharmacy software, and standard productivity applications.
• HIPAA-compliant home workspace is essential.
• High-speed cable broadband or fiber optic internet service with a minimum of 10 Mbps download and 5 Mbps upload is required.
