This is a fully remote position, open to applicants in California.
• Deliver comprehensive support to patients and healthcare providers to enhance patient care and medication adherence.
• Cultivate personalized relationships with patients and prescribing physicians within designated clinics.
• Offer medication management, reconciliation, dose optimization, and counseling on adherence.
• Conduct assessments related to disease states and provide drug-therapy education via phone, virtual, and video platforms.
• Identify, address, and prevent medication-related issues.
• Create tailored care plans in collaboration with interdisciplinary care teams.
• Oversee medication regimens to ensure therapeutic goals are met while monitoring for adverse effects, drug interactions, and clinical criteria.
• Suggest drug-therapy optimization strategies and initiatives to reduce medication costs.
• Engage in quality-improvement efforts, clinical outcome research, REMS compliance/reporting, and process enhancement projects.
• Provide clinical justification, supporting literature, and rationale for prior authorization requests and appeals.
• Collaborate with physicians, specialists, onsite pharmacists, healthcare providers, departments, and stakeholders.
• Assist with occasional peer-to-peer inquiries.
• Conduct targeted clinical assessments of drug levels and provide educational counseling.
• Ensure adherence to clinical compliance with FDA guidelines, clinical protocols, and evidence-based literature.
• Enhance workflows and implement strategies aimed at increasing efficiency, productivity, and the quality of care.
• Advocate for patient safety and medication literacy through education and counseling.
• Safeguard the confidentiality of organizational information.
• Participate in team meetings, conferences, and professional development activities.
• Stay updated on advancements in therapy management and integrate them into practice.
• Perform additional responsibilities as assigned by the Senior Director of Clinical Services and the Manager of Clinical Services.
• A Doctor of Pharmacy (PharmD) degree is required.
• A valid pharmacist license in California is necessary.
• Must be recognized as an advanced practice pharmacist in California.
• Must satisfy two of the three advanced practice pharmacist criteria: current relevant certification, completion of a U.S. postgraduate residency, or 1,500 hours of clinical experience under a collaborative practice agreement/protocol within the past 10 years.
• Strong understanding of pharmacology advancements and FDA health regulations.
• Excellent technical and clinical skills are required.
• Knowledge of disease-state management and treatment protocols is essential.
• Ability to analyze data and make sound judgments is crucial.
• Strong verbal communication skills are necessary.
• Ability to manage processes while ensuring legal compliance.
• Proficiency in Microsoft Office applications, including Outlook, Teams, Word, Excel, and PowerPoint.
• Competence in navigating internet and web-based platforms.
• Data entry and computer skills will be assessed during the pre-employment process.
• Must be organized, reliable, empathetic, detail-oriented, quality-focused, and highly motivated.
