Clinical Pharmacist – Medicare STARS

This is a fully remote position, open to applicants in Alabama, +25 more states.

📋 Description

• Exhibits extensive understanding of current pharmacotherapeutic principles, medical terminology, pathologies, disease management, age-appropriate therapy, and other relevant information related to medical and/or pharmaceutical care management.

• Collaborates with Sentara Health Plan (SHP) members to address medication-related care gaps.

• Handles referrals from the team.

• Provides counseling to members regarding their medication therapy.

⛳️ Requirements

• BLD - Bachelor's Level Degree in Pharmacy is REQUIRED.

• Pharmacist License (RPh) - State license - Other/National is REQUIRED.

• Proficiency with online pharmacy adjudication systems is REQUIRED.

• Advanced certification in HIV, oncology, infectious disease, or ambulatory care is preferred.

• Experience with Medicare STARS is highly preferred.
