Clinical Pharmacist – HEDIS Medication Adherence

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Spearhead initiatives aimed at enhancing HEDIS medication adherence and various pharmacy quality metrics, including adherence to antihypertensive, cholesterol-lowering, and diabetes medications.

• Evaluate medication adherence data, pinpoint care gaps, and execute targeted interventions to elevate adherence rates.

• Partner with healthcare providers, care managers, and other stakeholders to formulate and implement strategies that foster adherence to prescribed treatments.

• Conduct medication therapy management (MTM) sessions to tackle barriers to adherence, deliver patient education, and ensure comprehension of medication protocols.

• Engage directly with patients to identify and resolve medication adherence challenges, including issues related to cost, side effects, and complex dosing schedules.

• Apply motivational interviewing techniques to inspire patients to stick to their prescribed medication regimens.

• Track and monitor HEDIS performance metrics, generating reports to evaluate the success of interventions and programs.

• Leverage electronic health records (EHR) and pharmacy data to identify patients at risk of non-adherence and devise action plans.

• Provide consistent updates to leadership on progress towards HEDIS objectives and offer recommendations for enhancements.

• Act as a subject matter expert (SME) on medication adherence within the organization.

• Deliver training and support to other pharmacists, care managers, and clinical staff on optimal practices in medication adherence.

⛳️ Requirements

• Doctor of Pharmacy (PharmD) degree from an accredited pharmacy school.

• Active pharmacist license in one or more of the following states: TX, PA, AL, TN.

• At least 2 years of experience in HEDIS-related medication adherence work, quality enhancement, or population health.

• Strong understanding of HEDIS measures, especially those concerning medication adherence.

• Proficiency in utilizing EHR systems and data analysis tools.

• Exceptional communication, problem-solving, and organizational abilities.

• Capability to work independently as well as collaboratively in a team environment.

• Experience with motivational interviewing or other patient engagement strategies is advantageous.
