This is a fully remote position, open to applicants in Texas.
• Perform thorough medication reviews and post-discharge medication reconciliation for members of Virtual Primary Care and Complex Care and Case Management.
• Work in partnership with the Virtual Primary Care team and patients using standing orders and medication therapy management to enhance chronic disease management outcomes.
• Suggest or initiate therapies, manage patient care over time, and modify treatment plans as necessary.
• Act as a subject matter expert for colleagues and members regarding pharmacy benefits navigation.
• Create and deliver educational content to members via group classes.
• Provide training and presentations for internal teams.
• Contribute to the overall pharmacy strategy with the pharmacy team and key stakeholders throughout the company.
• Collaborate with clinical leadership to devise innovative pharmacy interventions, evaluate internal and external pharmacy solutions, and improve population health outcomes.
• Offer pharmacy benefits navigation, treatment guidance, medication support, and chronic disease management to members.
• Partner with providers, nurses, social workers, behavioral health specialists, and care coordinators to provide evidence-based care.
• A valid pharmacist license in good standing with the Texas State Board of Pharmacy is required.
• Multiple state licenses are highly preferred, along with a willingness to obtain additional state licenses; preference is given to Arkansas, California, Washington, Colorado, Arizona, and/or Iowa.
• Doctor of Pharmacy (PharmD) from an accredited institution.
• Board Certification in Ambulatory Care or Pharmacotherapy (BCACP or BCPS) is preferred.
• Over 5 years of experience in primary care, ambulatory care, or a comparable clinical environment.
• More than 2 years of experience in specialty pharmacy and high-cost medications.
• Clinical experience in primary/ambulatory care within an integrated health care system and/or health plan is required.
• Availability to work business hours in the PST or MST time zones is preferred, Monday through Friday from 8 am to 6 pm; specific hours are flexible.
