This is a fully remote position, open to applicants in Pennsylvania.
• Deliver comprehensive medication management for patients dealing with chronic conditions.
• Perform telehealth or relevant in-person consultations to evaluate medication effectiveness, safety, adherence, and patient comprehension.
• Collaborate with physicians, advanced practice providers, nurses, and care coordinators to create personalized care plans.
• Educate patients about their disease conditions, medications, and strategies for self-management.
• Ensure the appropriateness of drug therapy, accuracy in dosing, and cost-effective medication choices.
• Analyze laboratory and clinical data to formulate treatment plans and monitor therapeutic responses.
• Provide evidence-based recommendations to the overseeing medical team.
• Manage patients in accordance with collaborative practice agreements, which includes the initiation, adjustment, and cessation of medications where permitted.
• Identify and engage high-risk or high-cost patients utilizing population health tools, risk stratification reports, and EMR dashboards.
• Contribute to quality improvement initiatives centered on chronic disease metrics.
• Work with care teams to minimize hospital readmissions and emergency department visits.
• Develop and maintain clinical protocols, workflows, and documentation templates.
• Monitor and report on clinical outcomes, intervention metrics, and quality measures.
• Act as the medication authority within the primary care team.
• Communicate clinical interventions and care plans through EMR systems and team huddles.
• Engage in care team meetings and provider education sessions.
• Collaborate with external healthcare professionals and organizations to guarantee continuity of care.
• Bachelor of Science in Pharmacy or Pharm.D. degree.
• Current and unrestricted Pharmacist license in the practicing state, to be obtained within 60 days of employment.
• Capability to acquire licensure in additional states as necessary.
• Completion of a PGY1 pharmacy residency or at least 3 years of clinically relevant experience.
• Potential to achieve Board Certification (BCACP, BCPS, BCGP) within one year of meeting eligibility criteria.
• Legal authorization to work in the United States.
• Willingness to travel up to 20% of the time based on business requirements.

• Preferred: Over 2 years of experience in an ambulatory care, primary care, or population health environment.
• Preferred: Active Board Certification (BCACP, BCPS, or BCGP).
• Preferred: Completion of a PGY2 pharmacy residency.
