Clinical Documentation Specialist – Session Checker

This is a fully remote position, open to applicants in Philippines.

📋 Description

• Review daily service sessions and their related documentation to ensure they are complete and accurate.

• Confirm session dates, times, service details, providers, and other necessary information.

• Examine session notes and supporting documents for any missing or inconsistent information.

• Detect documentation errors, discrepancies, or incomplete records and collaborate with the relevant staff to implement corrections.

• Adhere to established protocols for verifying sessions and reviewing documentation.

• Monitor pending, completed, rejected, or corrected sessions.

• Keep precise records of verification activities and unresolved issues.

• Report recurring documentation or compliance issues to the appropriate supervisor.

• Collaborate with clinical, scheduling, administrative, and billing teams whenever clarification is necessary.

• Manage documentation with confidentiality according to applicable privacy standards.

• Support internal audits and quality-control assessments as required.

• Observe recurring issues and recognize opportunities to enhance documentation accuracy and workflow efficiency.

• Achieve daily or weekly productivity and accuracy goals.


⛳️ Requirements

• A bachelor's degree in Healthcare Administration, Psychology, Business Administration, Human Services, or a related discipline is preferred.

• 1–2+ years of experience in healthcare administration, medical/clinical documentation, ABA, behavioral health, insurance, billing, or quality assurance is preferred.

• Experience in reviewing clinical or healthcare documentation is highly desirable.

• Familiarity with ABA terminology, therapy sessions, or behavioral health documentation is a plus.

• Exceptional attention to detail and high data-entry accuracy.

• Strong organizational and time-management capabilities.

• Ability to identify discrepancies and ensure issues are resolved.

• Comfortable handling repetitive, detail-oriented quality-control tasks.

• Excellent written and verbal communication skills.

• Proficient with spreadsheets, databases, and standard office productivity applications.

• Ability to manage confidential patient/client information appropriately.

• Comfortable working under deadlines and with high volumes of records.
