
Clinical Case Manager, Behavioral Health

This is a fully remote position, open to applicants in Pennsylvania.
• Deliver telephonic clinical case management services for the Medicare demographic.
• Utilize Motivational Interviewing and various engagement techniques to improve member involvement in case management initiatives.
• Conduct a Comprehensive Assessment and develop a tailored Plan of Care.
• Record all interactions in clinical systems that support legacy Aetna and Coventry memberships.
• Provide behavioral health consultations and work in partnership with internal Aetna teams.
• Actively participate in clinical treatment rounds.
• Engage in team-driven initiatives aimed at program development—innovative ideas are welcomed.
• Allocate the majority of work hours to telephonic outreach and computer-based documentation tasks.
• Assist members in finding suitable community-based behavioral health resources.
• A minimum of 3 years of direct clinical practice experience.
• An active and unrestricted clinical behavioral health license in your state of residence (e.g., LPC, LSW, LCSW, LMFT, LISW).
• Experience working in behavioral health as part of a multidisciplinary team supporting the Medicare population.
• Excellent time-management and organizational abilities, with proficiency in multitasking in a dynamic environment.
• Capability to remain seated for extended periods while completing computer-based documentation.
• Experience with Motivational Interviewing is advantageous.
• Reliable access to a residential broadband internet connection of at least 25 Mbps download and 3 Mbps upload.
