This is a fully remote position, open to applicants in New Mexico.
• Establish and maintain communication with assigned and active members through video, phone, text, and email.
• Gather and apply clinical information to create a customized member care plan and clinician-centered care plan following NCQA guidelines.
• Evaluate members' health status and their care coordination requirements.
• Collaborate with the healthcare providers of members as necessary.
• Use motivational interviewing techniques, tailored care plans, and the Triple Aim framework to empower active members in achieving their health objectives.
• Clearly explain the benefits of our Program to new or inactive members, particularly for those with chronic kidney disease and those at risk.
• Properly document, request, and transmit member information in compliance with HIPAA regulations.
• Meet individual performance metrics to enhance member engagement and retention.
• Must possess a valid RN license in good standing and be open to obtaining licensure in other states.
• A minimum of five years of RN experience is mandatory.
• Must reside in PST or MST and be available to work at least three late shifts per week.
• Experience in developing care plans for individuals with chronic conditions and/or Chronic Kidney Disease is strongly preferred.
• Bilingual proficiency in Spanish is highly preferred.
• A BSN degree is strongly preferred.
• CDE or CDCES certification is highly preferred.
• Previous experience working 100% remotely as a nurse is highly preferred.
• Willingness and ability to travel occasionally, which may include overnight stays for corporate events, conferences, and health fairs.
• Capability to attend and engage professionally in video meetings.
• Proficient technological skills, with the ability to effectively and efficiently use computers, peripheral equipment, and applications/systems, including Microsoft products.
• An autonomous self-starter who is comfortable navigating ambiguity.
• A creative mindset with the ability to appropriately challenge the status quo.
• Excellent written and verbal communication skills.
• Capability to overcome challenges with a ‘yes if...’ attitude.
• Ability to effectively manage competing deadlines while maintaining focus on the overall objectives.
