This is a fully remote position, open to applicants in United States.
• Engage and connect with patients through video messaging, video consultations, phone calls, and text communication.
• Educate and enroll individuals in the medication adherence program.
• Provide personalized education, coaching, case management, and adherence support utilizing video and digital communication.
• Perform video-based medication reconciliation consultations.
• Review patient medication videos to confirm administration, monitor adherence, and identify any side effects or concerns.
• Develop short educational and motivational videos for patients, along with population-level health education.
• Evaluate medical, behavioral, and social needs, escalating cases when necessary.
• Offer guidance on medication administration, inhaler techniques, chronic disease management, and medication safety.
• Act as a clinical escalation resource for non-clinical team members.
• Assist patients with technical troubleshooting related to the mobile application and adherence platform.
• Ensure accurate documentation in line with company and regulatory standards.
• Coordinate with healthcare providers, health plans, and pharmacies on behalf of patients.
• Refer patients to healthcare providers and community resources, including facilitating warm transfers.
• Utilize motivational interviewing techniques to promote long-term adherence strategies.
• Provide patient insights to aid in program improvement.
• Collaborate with cross-functional teams to create video content, clinical protocols, and educational materials.
• Must be a legal resident of Puerto Rico.
• Must possess a Registered Nurse (RN) license in good standing.
• A minimum of 5 years of nursing experience is required.
• At least 5 years of relevant experience in patient engagement, health education, or care coordination.
• Fluency in English and Spanish, including reading, writing, speaking, and comprehension, is essential.
• Strong expertise in chronic disease management, medication adherence, and patient coaching.
• Excellent oral and written communication skills, delivered with a warm and empathetic approach.
• Ability to thrive in a fast-paced, patient-centered environment.
• Proficient in electronic health records (EHR), mobile applications, and digital health platforms.
• Comfortable engaging with patients via phone, video, and chat throughout the workday.
• Experience evaluating social determinants of health and linking patients with resources.
• Availability for evening and weekend shifts is required.
• Must be legally authorized to work in Puerto Rico/United States without employer sponsorship; visa sponsorship is not available.
