Case Manager, Social Worker - Field

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Oversee care for members requiring long-term rehabilitation and/or psychosocial support

• Execute and coordinate case management, psychosocial, and quality-of-life initiatives throughout the care continuum

• Organize consultant referrals, home care visits, community resources, and alternative care levels

• Formulate and implement strategies that integrate both acute and long-term care services

• Deliver psychosocial interventions through resource identification and program creation

• Manage members with acute or chronic conditions, emphasizing quality of care, compliance, outcomes, and cost-effectiveness

• Conduct initial and ongoing assessments of members to ascertain treatment pathways

• Create personalized case plans for members at risk of unfavorable outcomes

• Relay patient updates and facilitate communication with providers, physicians, Medical Directors, pharmacists, and care management teams

• Identify essential care available under member benefit plans in home or community environments

• Offer information, resources, and education regarding social issues, mental health, and available services

• Design programs aimed at reducing admissions, enhancing community integration, and improving health outcomes

• Mentor junior colleagues in social work techniques, processes, and responsibilities

• Partner with community members, organizations, and policymakers to tackle community challenges and enhance awareness

⛳️ Requirements

• At least 2 years of experience in medical social work or case management

• Must adhere to all state regulations relevant to the location of job responsibilities

• Willingness and ability to travel 25%-50% of the time within Wayne and Macomb Counties

• Reliable transportation is necessary

• Basic understanding of problem-solving and decision-making skills

• Fundamental knowledge of digital literacy

• Basic familiarity with medical terminology

• Ability to interact tactfully with customers and community members

• Capability to handle sensitive information in an ethical and responsible manner

• Skill in weighing the costs and benefits of potential actions to select the most suitable option

• Competence to operate within a clinical setting that encompasses diverse cultural dynamics among staff and patients

• Bachelor’s Degree in Social Work as mandated by state law

• LLBSW, LBSW, LLMSW credentials

• Behavioral health experience is preferred

• Bilingual proficiency in Spanish or Arabic is preferred
