This is a fully remote position, open to applicants in Massachusetts.
• Evaluate, plan, facilitate, coordinate, assess, and advocate for various care options and services.
• Address the comprehensive health needs of individuals and families through effective communication and available resources.
• Encourage quality outcomes that are also cost-effective.
• Conduct in-person case management activities within a specified geographic area as directed or required by business needs.
• Collaborate with members, families, and accessible care resources.
• Manage multiple systems and applications concurrently.
• Communicate via email, calendar invites, Teams messaging, and virtual meetings.
• Access secure systems, including VPN and EHR portals.
• Resolve common technical issues and reach out to IT support when necessary.
• Active and unrestricted Compact license from Massachusetts.
• Registered Nurse with a valid state license in good standing within the region where duties are performed.
• Preferred minimum of 3–5 years of clinical practical experience in areas such as diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac care with Medicare members.
• Minimum of 2–3 years of experience in case management, discharge planning, and/or home health care coordination.
• Ability to occasionally travel within a designated geographic area for in-person case management activities.
• Bilingual candidates are preferred.
• Strong analytical and problem-solving abilities.
• Excellent communication, organizational, and interpersonal skills.
• Capability to work independently.
• Proficiency in MS Word, Excel, Outlook, PowerPoint, and proprietary applications.
• Competent computer skills, including navigating multiple systems and keyboarding.
• Willingness and capability to obtain multistate RN licenses if necessary; company support will be provided.
• Bachelor's degree or equivalent experience; a bachelor's degree must be achieved within 3–5 years as part of role development.
• Certified Case Manager credential is preferred.
• Additional national professional certifications (CRC, CDMS, CRRN, COHN, or CCM) are preferred but not mandatory.
• Direct/hardwired internet connection to a modem/router within 7 feet of the computer is required.
• Minimum internet speed of 25 Mbps download and 3 Mbps upload is necessary.
• A quiet, secure, private, designated home workspace that complies with CVS Health and HIPAA guidelines is essential.
• Must operate within the state and city of current residence.
• Ability to utilize multiple monitors and applications simultaneously.
• Capability to use Microsoft Office 365 applications or similar tools like Google Workspace.
• Ability to log into secure systems, utilize VPN and EHR portals, manage strong passwords, and identify suspicious emails or links.
• Skill in resolving common technical issues independently.
• Willingness to learn new workplace technology skills.
